ABSTRACT

Objective @#To investigate equity of resource allocation in ophthalmology departments of medical institutions in Shenzhen City, so as to provide insights into the optimization of resource allocation in ophthalmology departments in Shenzhen City.@* Methods @#The numbers of beds and ophthalmologists in ophthalmology departments of medical institutions in Shenzhen City were collected through the Shenzhen Health Statistical Yearbook 2019. The distribution of resources and equity of resource allocation were evaluated in ophthalmology departments of Shenzhen City using Lorenz curve and Gini coefficient.@* Results @#There were 5.95 beds and 4.62 ophthalmologists in ophthalmology departments per 100 000 permanent residents in Shenzhen City in 2019. There were 13.35 beds and 9.51 ophthalmologists in ophthalmology departments per 100 000 permanent residents within the former special zone (Luohu, Futian, Nanshan and Yantian districts), and 2.17 beds and 2.13 ophthalmologists in ophthalmology departments per 100 000 permanent residents outside the former special zone (Guangming, Baoan, Longhua, Longgang, Pingshan districts and Dapeng New Area). The Gini coefficients of beds and ophthalmologists in ophthalmology departments were 0.348 and 0.243 by permanent residents in Shenzhen City, 0.386 and 0.386 within the former special zone and 0.086 and 0.012 outside the former special zone, respectively. The Lorenz curves of beds and ophthalmologists in ophthalmology departments were closer to the equity line outside the former special zone in relative to within the former special zone. @* Conclusion@#The gross number of beds and ophthalmologists remains to be increased in ophthalmology departments of medical institutions in Shenzhen City, and the equity of regional resource allocation is poor, which is mainly characterized by resource scarcity in ophthalmology departments outside the former special zone.

ABSTRACT

In order to better assist doctors in the diagnosis of dry eye and improve the ability of ophthalmologists to recognize the condition of meibomian gland, a meibomian gland image segmentation and enhancement method based on Mobile-U-Net network was proposed. Firstly, Mobile-Net is used as the coding part of U-Net for down sampling, and then features are extracted and fused with the features in decoder to guide image segmentation. Secondly, the segmentation of meibomian gland region is enhanced to assist doctors to judge the condition. Thirdly, a large number of meibomian gland images are collected to train and verify the semantic segmentation network, and the clarity evaluation index is used to verify the meibomian gland enhancement effect. The experimental results show that the similarity coefficient of the proposed method is stable at 92.71%, and the image clarity index is better than the similar dry eye detection instruments on the market.

ABSTRACT

@#AIM: To explore the effects of sevoflurane combined with dexmedetomidine on emergence agitation(EA)in children under went general anesthesia in ophthalmology department. <p>METHODS: Totally 84 children underment elective ophthalmic surgery in the hospital from May to December 2018 were selected as the research objects. They were randomly divided into study group and control group with 42 cases in each group. All patients in haled sevoflurane to maintain general anesthesia. After anesthesia intubation, children in the study group were treated with intravenous injection of 0.5μg/kg of dexmedetomidine in a constant-speed while children in the control group were treated with same volume of normal saline. The monitoring data of heart rate(HR), mean arterial pressure(MAP)and serum stress response indexes such as norepinephrine(NE)and cortisol(Cor)during recovery period were obtained. The anesthesia recovery, EA and occurrence of complications were recorded. <p>RESULTS: There were obvious fluctuations of HR and MAP in the control group while awake, at 1min, 5min and 10min after extubation. There were significant differences at different time points(<i>P</i><0.05), while there was no significant difference in study group(<i>P</i>>0.05). HR and MAP were significantly lower than those in control group at each time point(<i>P</i><0.05). Levels of serum NE and Cor in study group while awake and at 10min after extubation were significantly lower than those in control group(<i>P</i><0.05). There was no significant difference in spontaneous breathing recovery time or awake time between the two groups(<i>P</i>>0.05). Compared with the control group, the extubation time was significantly shorter and score of extubation quality was significantly lower in the study group(<i>P</i><0.05). The incidence of EA in study group was significantly lower than that in control group(12% <i>vs </i>31%)(<i>P</i><0.05). There was no significant difference in the severity of EA or incidence of complications(<i>P</i>>0.05). <p>CONCLUSION: Sevoflurane combined with dexmedetomidine can effectively stabilize hemodynamics of children under went general anesthesia in ophthalmology department during recovery period, and reduce stress response, thus effectively reducing the incidence of EA, without affecting anesthesia recovery quality and with high safety. However, excessive sedation should be payed attention to.

ABSTRACT

A therapeutical intervention in 100 patients operated of strabismus, assisted in the Ophthalmology Department of the Southern Children Hospital in Santiago de Cuba, was carried out from October, 2006 to the same period of 2013, in order to evaluate the effectiveness of the homeopathic treatment in them. The affected ones were divided in 2 groups of 50 members each: the study group which received homeopathic treatment, and the control group, treated conventionally. There was no significant difference as for age and sex in both groups (p>0.5). After applying the therapies a marked effectiveness of the homeopathic treatment was obtained (96.0%) in relation to the conventional one, with a significant difference between both groups p <0.5.

ABSTRACT

A prospective analytic case-control study was carried out in children with glaucoma or strabismus or both, treated at the Department of Ophthalmology of the Southern Children Hospital in Santiago de Cuba, from January to June 2011, with the purpose of assessing the visual perception of coherent motion in them. The case group included 72 randomly selected patients, and control group with the same number of members who had all their vision. It was determined that children with visual disorders made more errors, omissions and fewer hits, besides being less rapid in solving oriented tests, as they had longer reaction time to do them.
